Frequently Asked Questions

How many times a year can I have my teeth cleaned?
You are NOT limited in the number of times you can visit your dentist.

Q: What is work in progress? Is it discounted?
Work in progress is any oral health procedure that is started and not completed prior to you enrollment in a dental program. Work in progress is not covered.

If any work is in the process of being done, you cannot go to one of our dentists to complete the work at the discounted rate.

Q: Are cosmetic procedures discounted? If so, what procedures?
Porcelain laminate veneers are listed on our fee schedule under “Restorative Procedures.” Chair side bleaching is available at a 25% discount off of the doctor’s usual and customary fee (Britesmile is not a covered procedure).

Q: Is Orthodontia discounted? What are the Orthodontia services? If yes, is Invisalign braces discounted?
Orthodontia is listed on the fee schedule for children up to and including age 16 at a significant savings. It is available to those members over age 16 at a 25% discount. Invisalign braces are 25% off of the usual and customary fee of the participating provider.

Q: Do I need to obtain Claim Forms?
There are NO claim forms to fill out.

Q: Is this insurance?
No. The Patriot Dental Plan is a reduced fee for service plan. There is no form of payment from Patriot Dental to the dentists.

Q: Can I change my dentist once I am in the plan?
You many change your dentist within the network whenever you wish.

Q: My dentist isn't currently in the directory. What can I do?
We will contact your doctor to see if he/she would like to join. Patriot Health is always looking to expand our panel. Simply call our customer service department and give the doctor’s name, address, phone number and specialty and we will call our doctor immediately.

Q: How long does it take to process my application?
It will take approximately 48 hours before you will be able to use the program.

Q: When will I receive my membership package and what will it include?
You will receive a welcome package with the nearest participating dentists, a fee schedule and a Patriot Dental card. (Allow 1-2 weeks for delivery of subscriber cards).

Q: If I choose to pay by credit card, what will my credit card statement read?
Your credit card statement will read “Patriot Health, Inc.”

Q: If I choose to pay by Bank Account Draft (ACH), what will my bank statement read?
The statement will read “Patriot Health, Inc.”

Q: Is the processing fee billed separately or with the plan payment?
The one-time non-refundable processing fee is billed with the first prepayment fee.

Q: Can I change my payment type once I am in the plan?
You can change the payment method at any time but it must stay consistent from that point on.

Q: If I started with monthly payment, can I change it to annual payment at a later date?
You may change from monthly to annual by calling our Customer Service Department.

Q: Do the applicant name and the billing name need to be the same?
No. However, we do need the information of the person who is being billed/charged, i.e. full name payment information and address.

Q: What if I need to change my membership from Individual to Family?
You can upgrade at any time and will be charged/billed accordingly.

Q: how do I add or cancel membership for additional members?
You may upgrade to include a dependent or downgrade and remove a dependent. You will then be charged accordingly to reflect the change.

Q: If I cancel the policy, will I receive a refund of the payment or processing fee?
The processing fee is non-refundable.
