The Guardian dental plan gives you the freedom to receive dental treatment from any dentist You choose.

Reimbursement is based on the lower of your dentist’s fees or the amount that would be paid to dentists who have agreed to be reimbursed according to our negotiated fee schedule.

If you choose to see a dentist outside of the Network, you will be responsible for the deductible and any amounts over the “fee schedule” as well as any co-insurance.
