A usual, customary, and reasonable fee (UCR) is the amount of reimbursement an indemnity dental insurance plan determines is appropriate for a dental service covered by the plan. Since this is an insurance company-specific definition, the UCR reimbursement amount may or may not reflect the actual cost of dental services within the region in which the patient lives.
