Ineligible Provider

An ineligible provider is a dental care professional who is not entitled to reimbursement under the terms and conditions of a specific dental plan. Even if a patient receives care covered under his or her dental plan, the insurance company will not pay for it if that care is delivered by an ineligible provider. HMO dental plans typically consider out-of-network dentists as ineligible providers.
