Q: How many times a year can I have my teeth cleaned?
One cleaning every 6 months.
Q: What is a pre-existing condition? Is it covered?
A pre-existing condition is an oral health condition, which existed before your enrollment in a dental program. The plan does not include an exclusion for pre-existing conditions
Q: What is work in progress? Is it covered?
Work in progress is any oral health procedure that is started and not completed prior to your enrollment in a dental program. The plan does not cover any expenses prior to the date you are covered under the Policy.
Q: Are cosmetic procedures covered? If so, what procedures?
There are no benefits for cosmetic procedures.
Q: Is Orthodontia Covered? What are the Orthodontia benefits?
Yes, Coverage up to age 19 that escalates to 50% in the third year, with $150 lifetime deductible, and $1,200 lifetime maximum. Invisalign braces are not covered under this plan.
Q: Do I need to obtain claim forms?
If the dentist does not file the claim, the member must obtain a claim form and file the claim themselves.
Q: Is there a waiting period?
No, there are NO waiting periods with this plan.
Q: Is this insurance?
Yes.
Q: Can I change my dentist once I am in the plan?
Yes, you may change your dentist at any time.
Q: My dentist isn’t currently in the directory. What can I do?
You may want to call your dentist to confirm whether he is a Maximum CareTM dentist. If he does not participate in a Maximum Care network, he can charge potentially higher rates than Maximum CareTM dentist. This may likely affect how much more you pay out of pocket for procedures. To reduce your out of pocket expenses, you can select a dentist that is in the directory.
Q: How long does it take to process my application?
There is no deadline to enroll. Applications submitted by the 19th of the month can become effective on the 1st of the following month. Any applications received after the 19th can become effective on the 1st of the second month.
Q: When will I receive my new member kit and what will it include?
You will receive your enrollment package upon completion of enrollment and payment of applicable premiums/enrollment fees, or a few days prior to the selected effective date. The enrollment package will include your Certificate of Coverage and I.D. cards and is sent via email from noreply@morganwhite.com.
Q: If I choose to pay by credit card, what will my credit card statement read?

Q: If I choose to pay by Bank Account Draft (ACH), what will my bank statement read?

Q: Is the processing fee billed separately or with the premium?
The processing fee is billed at the same time the premium is billed. However, there will be 2 charges to your account when the initial premium is drafted, one for the processing fee, and one for the premium due.
Q: Can I change my payment type once I am in the plan?
Yes.
Q: If I started with monthly payment, can I change it to annual payment at a later date?
Yes.
Q: Do the applicant name and the billing name need to be the same?
No.
Q: What if I need to make changes to my coverage (example: add or remove a dependent/spouse)?
Call Morgan White, the administrator for the plan, at 1-866-569-8654 or log into the Client Portal at my.mwadmin.com. You may also email your request to individualchanges@ morganwhite.com or fax your request to (601) 956-3795. Please allow 3-5 business days for processing.
Q: If I cancel the policy, will I receive a refund of the premium or processing fee?
The policy can be cancelled by submitting a written cancellation request, as indicated in your policy. We will refund any premium paid, but not yet earned due to policy cancellation. The refund will be based on the number of full months that remain in the 12 month policy period. For example, if you choose to pay Annually, and you decide to cancel the plan after 10 months, you will receive a refund for 2 months premium. You will receive a refund of the premium only. The processing fee is non refundable.
